excellent care & valu e with excellent nursing staff

My dad was transferred to this facility after spending a week in the hospital recovering from a hip replacement and a broken femur. He was transported directly from the hospital and placed in a corner room that was extremely hot (at least 80 degrees), with paint chipping off the walls and a strong odor that suggested poor hygiene throughout the facility. There were even spiders on the floor. The most concerning issue was his pain management. The nursing staff either did not have or had lost his prescribed pain medication. Because it was the weekend, they told us there was nothing they could do. They actually allowed my mother to go home, retrieve two-year-old pain medication, and give it to him while he was a resident in their care. Several people who had previously worked at the facility told us they would not send their worst enemy there. After seeing the conditions firsthand and realizing my father was not receiving appropriate care, we called an ambulance and had him transported back to the hospital so he could be placed somewhere else.

They barely change briefs, i found my grandpa slumped over in his food, and to top it all off, they stole all his belongings when he went to the hospital for two days.

My mother, a dementia patient who had broken her hip, recently spent about a month rehabbing at Sullivan Park Care Center (skilled nursing facility). The staff at SP were wonderful! They were always upbeat and positive with mom, and kept a close eye on her. They also kept me, her caregiver, regularly updated and did a great job of coordinating her rehab and care with me. Her rehab went great and she is doing very well. Now that mom is back at her memory care facility, I am very glad that she was able to rehab at Sullivan Park. Thank you to all the staff there!

Friendly staff💐

This place is HORRIBLE! Our grandmother was there not even 48 hrs before we called the ambulance to get her out of there. The staff does not communicate with each other or know what is going on with new intakes. Our grandmother was very ill and had been in the hospital for a week prior to coming here. When she left the hospital she was talking and STABLE- after the time she spent here it was like she went backwards. They did not assist her with basic needs that were clearly stated on discharge paperwork. When we tried to ask questions about what went wrong the facility manager didn’t give any sympathy to our family or ask how my grandma was. She was practically dead when she went back to the hospital. Then the next day the office lady asked if we wanted to pay to hold her room for her to return. Are you joking?!?! All they care about is $$$$. Do not send your loved ones here! Period.

I would suggest anyone who need physical help come here. This is a blessed place. It has helped me get stronger without force or putting pressure on me. The food is good. The people are great. They help teach you how to take care of yourself. I hope whoever is reading this will come here. Thank you all for your time and patience. God bless Sullivan Park and the people who work here.

I can’t say enough good and positive things about Sullivan Park. All the way from check-in till discharge i was treated with the utmost respect and professionalism. Most importantly, they truly care! But, I especially want to thank all the nurses, physical therapy , associates and all my peeps in 100 Hall. I spent just under 30 days and the staff always came in every single day with a smile on their face willing to do whatever to make sure everyone was taking care of!

bed bugs recently. Understaffed. Overworked. I wouldn’t leave a stray dog in their care.

I’m a current patient for rehabilitation at Sullivan Park Care Center. They have met my every need. The are more than willing to help me and encourage me to do as much as I can (which is reasonable) myself. I couldn’t be happier with a facility.

Choosing a skilled nursing facility for a loved one, in my case my mother, is not an easy task. I chose Sullivan Park, and have no regrets. The best part about this SNF is their staff. From the nurses (Mom LOVED all of her nurses), to the CNAs, the therapists were all top notch, the physicians, and even the cleaning staff. I’d be remiss not to mention the receptionist, who greats everyone as if today is the best day ever. They all made her stay great and lessened my concerns and worry. I would give them a 5 star if not for the meals. I really do understand that they are having to feed a wide range of pallets who may not like seasoned or highly flavorful food and many with a reduced ability to chew. They did do a good breakfast and certainly served a well balanced meal and always had a salad option. All told, I would highly recommend this SNF.

These people are the greatest care team ever. They care about your family member. Their PT and OT are great.

The staff are happy! I have had one meal that I didn’t like. My only complaint is the lights being on at night.

I can’t say enough positive things about Sullivan Park. From the moment we arrived, the staff was attentive, compassionate, and truly dedicated to their patients’ well-being.

Jessica is very pleasant! Most of the clinical team was nice! The housekeeping staff are very friendly! The team did an amazing job getting me out in a timely manner! They could use more handicap buttons on doors and the kitchen could use some work.

This place has good staff! Therapy is amazing! The food is ok, need bigger portions! I would definitely come back if I need SNF.

I can tell you there is no better place to be. The staff are on there job I am a patient here for a broken hip the nursing staff the aids the therapist are all so kind and on there jobs very polite and go the extra mile if you or someone you love needs to someplace to go for care you should come here you won’t be sorry I can’t say enough about this place they treat you like family

Staff is friendly and helpful! The process to get things done is slow but it eventually gets done. Food is adequate but can be improved. All in all it is ran like a typical skilled nursing!
